We ask that you provide any documents or information to us prior to your appointment to save time during the meeting. Navigate to the sections below to see how you can best prepare for your appointment.

To be best prepared for your appointment, we ask that you provide us with the following information:

  • Copies of your tax returns from the past three years
  • Social Security Numbers and dates of birth for all dependents
  • W-2 forms for wages
  • 1099 forms for interest, dividends, retirement, security, unemployment, and other income

Other helpful information to provide during your appointment (as it applies to you) includes:

  • Year-end statements from mutual funds
  • K-1 forms from partnerships, corporations, and/or estates
  • Rental or self-employment income and expense
  • Purchase and sale information from anything sold during the year
  • All other statements of income
  • IRA year-end statements
  • Medical expenses
  • Records of estimated taxes paid
  • Property tax statements
  • 1098 forms for mortgage or student loan interest
  • Donations of money to charity
  • Donations of property to charity
  • Volunteer expenses and mileage
  • Amounts paid for higher education
  • Job related expenses
  • Investment related expenses
  • Childcare provider’s name, address, SSN, and amount paid

Partnerships, Corporations, S-Corporations, and Non-Profit Organizations

For new business clients with an existing business, we ask that you provide us with the following information:

  • Copies of the last 3 years of business income tax returns
  • Copies of the last 3 years of individual income tax returns (if you are bringing in 1040 preparation)
  • Copies of the last 3 years of the company’s financial statements

We may request additional documents in the future, including:

  • Copies of most recent Annual/Quarterly payroll tax returns
    •  Federal
      • Form W-3
      • Form W-2
      • Form 940
      • Form 941
    • State, Quarterly
      • ESD
      • L&I
      • PFML
    • Department of Revenue, Monthly/Quarterly/Annually
      • Excise tax returns
  • Most recent Federal, Book, and AMT depreciation schedule
  • Backup copy of QuickBooks
  • Bank/credit card statements, if not reconciled via QuickBooks
 
For new business clients who are forming a new business, we ask that you provide us with the following information:
  • Copies of the last 3 years of individual income tax returns (if you are bringing in 1040 preparation)

Other information to provide, if it is available at the time of your appointment, includes:

  • Article of incorporation/LLC
  • Federal EIN statement from the IRS
  • State registration for sos.wa.gov
  • UBI number statement from Department of Revenue
    • DOR Login info 
  • Copy of QuickBooks
  • Bank/credit card statements, if not reconciled via QuickBooks

Estates and Trusts

For new estates, we ask that you provide us with the following information:

  • A copy of the death certificate
  • A copy of the decedent’s will and/or a copy of trust agreement (if there is a revocable, living trust separate from the will)
  • A copy of Letters Testamentary
  • Listing of the decedent’s inventory with the fair market value of each item
  • Copies of the last 3 years of individual income tax returns
  • The federal EIN for the estate (a copy of a letter from the IRS regarding the EIN)
  • Full name, address, phone number, email address, and social security number of personal representative and beneficiaries
  • Attorney’s name, address, and phone number
 
For new trusts, we ask that you provide us with the following information:
  • A copy of the trust agreement
  • The federal EIN for the trust (a copy of a letter from the IRS regarding the EIN)
  • Full name, address, phone number, email address, and social security number of personal representative and beneficiaries
  • Trust Asset Inventory and Cost Basis
  • Attorney’s name, address, and phone number
